A 74 year old man is referred to you electively for consideration of surgery with a six month history of worsening dyspnoea on exertion, now NYHA Class II. Additionally, in the last fortnight, he has had two presyncopal episodes.

Previous stent to RCA 5 years ago
Type 2 diabetes on oral hypogylcaemics
Obesity with a BMI of 33.
Remote smoker
Current Medications:
Aspirin, Plavix, Metformin, Amlodipine
Angiography demonstrates the right coronary stent to be patent and a proximal LAD stenosis of 70%
