You are called to the cathlab by your cardiology colleagues.  A TAVI has just been performed successfully under conscious sedation.  Because of postprocedural haemodynamic instability, a TTE was done on the table that showed a pericardial collection and a pericardial drain was placed. Blood was aspirated from the drain, with an improvement in haemodynamics, but after an hour of observation blood continues to drain and now totals 700mls. There is ongoing intermittent haemodynamic instability.  The arterial sheaths are out, but venous sheath remains. 

You are informed it is a patient discussed at a Heart Team meeting you were present at; an 81 year old woman with long term steroid use for SLE who walks with a four wheel walker.
